At Texas Alzheimer’s and Memory Disorders, it is our desire to provide you with timely, quality care. In order to do so, please arrive on time and to make every scheduled appointment. We feel it is important for you to understand our policy as it relates to cancellations, no shows and late arrivals.

New Patients

Please be sure that you have completed the online New Patient Packet information. If you have accessed this, please let the office know when you make the appointment. This early arrival allows our staff to efficiently perform its task of registering you into our systems.

Insurance and I.D. Cards: Please bring your insurance card and driver’s license or picture I.D. If your insurance company requires that you have a referral for your first visit, please make sure that your referring doctor has provided this.

We will file your insurance claims on your behalf, and help you with questions or concerns regarding payment of your bill. If you would like to speak with our financial counselor about questions or concerns regarding your insurance coverage or bill, please don’t hesitate to ask for a consultation.

Medications: You should bring a complete, up-to-date list of all medications you take, or bring the bottles and packages with you.

Follow-Up Visits

For follow-up visits, we ask that you arrive on time. Should you arrive more than 15 minutes late, your appointment may be moved to a work-in status or rescheduled.


If you have a scheduled appointment time and need to reschedule or cancel, please notify our office at least 24 hours prior to your appointment. We will gladly reschedule you a more convenient appointment time.


In an emergency, it is best to go to the emergency room at your nearest hospital. The emergency room staff will contact us or the physician on call.
